The features seen in the stone's crown & pavilion are naturally occurring rough, unpolished surface areas. When cutting & polishing blue sapphires, it's a common practice to intentionally keep certain portions of the stone in the natural, rough, unpolished form. This is done to preserve the carat weight and maintain the stone's overall shape/symmetry. Such minor external features are commonly seen in most natural & unheated/untreated blue sapphires.
